Documents you need to make a claim

The following documents are the basic requirements which must be submitted to process a death claim.

1. Notarized copy of original death certificate issued by local registrar's office.

2. Notarized copy of citizenship of beneficiary as well as Insured.

3. Notarized copy of relationship certificate showing relationship between beneficiary & the Insured.

4. Death certificate from the hospital. (if death occurred in the hospital)

5. Police report and/or Autopsy report. (in case of accidental or suspicious death)

6. Claim form to be completed & signed by beneficiary.

7. Physician's statement to be completed signed & stamped by the last Physician who attended the Insured. (If required)

8. Detailed medical report issued by the Treating Physician stating previous medical history, date of onset/diagnosis of disease-causing death as well as exact date of onset of related risk factors (e.g.Hypertension, Diabetes Mellitus, CAD, Dyslipidemia, Ischemic Heart Disease etc.).

The following documents are the basic requirements which must be submitted to process a critical illness claim.

1. Claim intimation along with the relevant details.

2. Claim forms (to be completed and duly signed by the Insured) and Physician Statement (to be completed and duly signed by Treating Physician). 

3. Detailed medical report issued by the Treating Physician stating previous medical history, date of onset/diagnosis of current ailment as well as exact date of onset of related risk factors (e.g. Hypertension, Diabetes Mellitus, CAD, Dyslipidemia, Ischemic Heart Disease etc.)

4. All relevant X-rays/ CT Scans / MRIs / Lab tests / Reports (ECG, Echo, Cardiac Enzymes, Angiogram, Histopathology, Cytology etc. as applicable)
